Mid Sussex Sports Therapy Privacy and Data Protection Policy

Introduction: 

Mid Sussex Sports Therapy (MSST) takes data protection and patients privacy very seriously and has a data protection policy that is in line with the new General Data Protection Regulations (GDPR).

As part of our operations and service to our patients, it is necessary to collect personal and sensitive data from you and this policy explains how we collect, process and store that data. 

Information we collect:

Personal information that we collect may include, name, address, contact numbers, email address, referral source, marketing preferences, brief past medical history and history of present condition requiring sports and remedial massage and treatment.

The data is collected at initial assessment, where clients complete the consultation documentation and provide their consent to treatment. When a patient consents to treatment, they consent to our use of their information for treatment purposes. If the patient is under the age of 16, then a guardian or parent consent is required.

Personal information (name, contact details, appointment request details and newsletter sign up request) may also be collected via our website.

Use and storage of your information:

The personal data and subsequent treatment notes are required to provide a high quality service to patients. They are held securely and in accordance with GDPR, The Sports Massage Association (SMA) and The Society of Sports Therapists (SST) codes of conduct.

An accurate record of patient details and treatment notes are required by the SMA and SST codes of conduct and the standard retention period for these notes is 7 years and in the case of children, up until the child is 21 years of age.

Disclosure of your information:

Personal information will not be disclosed to any third party without obtaining prior consent, unless required to do so by law.

We do not share, sell or distribute your data to third parties.

Control of the use of your data:

Your contact details may be used to provide you with appointment information, appointment reminders and provide you with special offers or a newsletter from MSST, if you have given us permission to do this. You can change or revoke that at any time by contacting: info@midsussexsportstherapy.com.

You have the right to access your information that is held by us. You can do this by emailing info@midsussexsportstherapy.com and asking for a copy of your personal information and patient notes. In accordance with GDPR, we must supply this information within 1 month.

You have the right to request that we cease using your data or for your data to be erased. You can do this by emailing info@midsussexsportstherapy.com and requesting erasure of data. In accordance with GDPR, we must comply within 1 month.

Changes to this policy:
